Below, you can find everything that you need to get started as a new patient at our dental office, including essential forms and financing information. Your First Dental Visit
Before you arrive, our dental office will instruct you on how to fill out online forms and enroll as a new patient. We confirm your forms as well as your appointment date two days prior to when your visit is scheduled to occur. Once you arrive, you’ll be greeted by our front desk and our hygienists. From there, they will guide you to one of our operatories to have their medical history reviewed. We’ll also take blood pressure, complete charting, perform X-rays as needed, then perform a cleaning. Dr. Ostrand will come in to meet you, perform an exam, then discuss potential treatment options as necessary moving forward.
New Patient Forms
With the help of new patient forms, we are not only able to learn more about your medical history. We’re also able to better personalize your treatment based on your specific needs! For convenience, we’ve included the online forms on our website so you can fill them out on your own time.
Patient Portal
Now it’s even easier to get the information you need when you need it. Our patient portal allows you to get the details you need for a more informed dental visit every time!
